Patient forms are sent to patients via email with the ability to be completed online, meaning that you do not need to worry about remembering paperwork to bring to your appointment.

Additionally, patients will be provided with a "superbill" to their patient portal after each visit in the event that a patient wishes to submit the superbill to insurance for potential reimbursement (no guarantee)

Concierge Style means patients have more direct access to Dr. Gallardo and will be explained at the first appointment.
